I agree to pay $195.00 for 5 hours of group consultation for Weekend 1 Basic Training.

I understand that there are no make-up sessions, refunds, or switching of groups if I miss one of the group sessions for which I am registered.

I understand that this consultation is by teleconference only and that I need to have my manual from Weekend 1 available for reference.

I understand that I must provide a copy of my certificate of completion for EMDR Basic training prior to participating in this consultation. I further understand that failure to provide proof of completion will result in cancellation of this session and be subject to fee's.

Conflict Resolution: A friendly atmosphere is advocated and will be fostered in the consultation process. If differences arise, Carolyn Settle will commit herself to resolve any issues in a professional and mutually beneficial manner, including, if necessary, bringing a third party to help in conflict resolution. I agree to abide by the code of ethics of the professional organization(s) to which I belong.
